PTA REIMBURSEMENTS

REIMBURSEMENT SUBMISSIONS

For reimbursement for PTA related expenses please complete the reimbursement request form.

  • Complete form information

  • Must be approved by budget line item, staff member or PTA Committee Chair. Forward to correct person for signature prior to submitting to the treasurer.

  • Scan receipts to pdf format (using any phone scanning app such as Dropbox, Adodbe Scan or CamScanner)

  • Email reimbursement form and receipts to:treasurer@mabearspta.org


Do not leave reimbursement requests in PTA cubby in the office.

REIMBURSEMENT CHECKS

Checks will be run approximately every two weeks. 

  • Checks can be mailed to recipient

  • Checks can be placed in PTA mailbox in the M-A Office

TAX INFORMATION

For the M-A PTA’s group tax exemption number, please see this letter confirming our  501(c)(3) status.

The group exemption number assigned to the California State PTA (our parent organization) is #23-7059344.
