PAYMENT/REIMBURSEMENT FORM
For payment or reimbursement for PTA expenses, please complete the online reimbursement form.

Expense must be budgeted. Please review form for appropriate budget line item.

Scan receipts to pdf format (using any phone scanning app such as Dropbox, iOS Files, Adobe Scan, or CamScanner) and upload using the form.

Checks will be run on a monthly basis.

Checks can be mailed to recipient or placed in PTA mailbox in the M-A Office.
